FYSA Day with the Rays is Sunday, September 21 at 1:40 p.m vs. the Minnesota Twins at Tropicana Field, St. Petersburg.

Affiliation Updates
REVISED 7/15/08
This is a reminder that all 2007-2008 affiliation forms must be up to date as we approach the Annual General Meeting (AGM). The cut off date for all 2007-2008 affiliation updates will be July 31st, 2008. If you have had elections, or for some other reason have any changes for 2007-2008, please contact Xiomy Gonzalez at xgonzalez@fysa.com for further instructions.
 
Information used for credentials at the AGM is based off of the 2007-2008 affiliation form, NOT the 2008-2009 form.   The affiliation form can be found on the secure portion of the FYSA website and can be accessed by the affiliate Agent of Record only.

There will be a workshop conducted on Saturday during the AGM weekend to discuss the proposals and hopefully answer any questions you may have before voting on Sunday.
 
Please review this document carefully to minimize the time spent on this on Sunday.
 
To those affiliates and members who submitted suggested changes, FYSA thanks you.
 
Bylaw changes require a two-thirds (2/3) majority vote to pass; rule changes require a simple majority vote (50% +1) to pass.

Affiliation Renewal and Registration Guidelines for the 2008-2009 Season
While the 2008 season seems so far away, it is time to begin making the plans necessary for the registration of all of our members and players.
 
The 2008-2009 online affiliation renewal form will be available on June 1 for all current affiliates. This form can only be accessed and completed by the affiliate’s Agent of Record. This form must be submitted by September 1, 2008 or all access to the secure site will be denied.

US Youth Soccer Disciplinary Action & Risk Management Report (DAR)
The US Youth Soccer Disciplinary Action and Risk Management Report (“DAR”) is a tool provided by the national organization to track suspensions of individuals participating in US Youth Soccer programs.  This information is updated and provided monthly by the 55 national state associations.  The DAR can be used in conjunction with the FYSA suspension list.  To view the current US Youth Soccer DAR report, click here.
